Injury Management


The club has partnered with Evado Studios located at 165 Burwood Rd, Hawthorn. Evado Studios are run by qualified Sports Physiotherapists and one of their physios will be at training on Tuesday nights to assess injuries.

All injured players are required to be at training on Tuesday nights to be assessed by the physio and given a rehab program. If it is necessary, you may be directed to make an appointment with the physio to further treat your injury. Evado will subsidise physio appointments for all Hawks players so you won’t be too out of pocket.


Evado also provide fitness classes on site and all Hawks members can join for $22 per week for unlimited classes (usually $35). Classes offered are Fitness Pilates, Clinical Pilates, Functional Strength, Conditioning Circuit, Spin classes and Boxercise. Classes run from 6am to 8pm so you can fit them in around work. The classes are small group training with up to 8 people in each class. You can book classes and physio appointments on the Evado Studios App.

Player Insurance

Players registered with an affiliated AFL Club enjoy the benefits of coverage under a Personal Injury policy, however there are three very important points players should be aware of.

What are you covered for?


‘Non-Medicare’ means medical costs that can’t be claimed through Medicare.

Items include physio, chiro, dental, ambulance transport and private hospital accommodation.


It is law that Medicare items can’t be claimed on Personal Injury cover. These items can be claimed on Private Health Insurance and include services such as doctors and surgeons.


Personal Injury cover is a policy for injuries occurring due to an accident. It is not a sickness or illness policy.


The Personal Injury cover starts at a standard level called Bronze.

This includes:

  1. 50% reimbursement of non-Medicare expenses

  2. $2,000 maximum per claim

  3. $100 excess

  4. Capital Benefit maximum $100,000

  5. Quadriplegia/Paraplegia maximum $1,000,000


It is strongly recommended that all players investigate their personal insurance needs.

Players should consider the benefits of:

  • Private Health insurance

  •  Life Insurance

  •  Loss of Income (particularly for self-employed players)

  •  Ambulance Subscription

MAKING A CLAIM - Personal Injury Claims

Option 1 – Complete Online

Submit your claim online.

Complete and submit all personal information and injury details relating to your claim online.

Following your online claim lodgement, JLT Sport will email you 3 separate Declarations that need to be completed and signed by your Club, your treating Physician and/or your Employer.

The Declarations can be printed and filled in manually or emailed straight to the relevant person for them to print, complete and return to the Insurer on your behalf.

Option 2 – Complete Manually

  1. Download the Personal Injury Claim Form 

  2. Complete all relevant sections of the form.

  3. Send your completed form to Echelon as soon as possible (and within 270 days from the injury date).

  4. Echelon will confirm receipt of your claim and provide you with a claim number.

  5. Any further costs can be submitted to Echelon quoting this claim number.

  6. Documents can be submitted by email, post or fax.

Important Information

The Health Insurance Act (Cth) 1973 does not permit the Trustee or Insurer to contribute to any charges covered by Medicare (including the Medicare Gap).

Personal Injury coverage through your club is limited by the Health Insurance Act. As such, JLT Sport encourage participants of AFL football to investigate the benefits of private health insurance.

To assist clubs, ensure players are aware of the benefits of private health insurance, there is a Private Health Audit available from the Downloads section of this website.

*Please note: MRI scans are generally claimable through Medicare, however sometimes the referrer and/or provider is not registered with Medicare. Please check with your treating physician prior to lodging your claim.

In addition, there will be no refund in respect of:

  • Any expenses recoverable by You from any other insurance scheme or plan providing medical or similar coverage or from any other source except for the excess of the amount recoverable from such other policies/plans.

  • Any expense to which the National Health Act (Cth) 1953 or any of the regulations made there under apply.


Please Note, if you are receiving benefits from player Insurance, as soon as you play a game the benefits STOP!!!!!!
